Position Summary : We are seeking a highly skilled and compassionate Family Practice Nurse Practitioner (FNP) to join our growing healthcare team. This position involves working at multiple office locations within Hudson County, providing comprehensive primary care services to patients across the lifespan, from infants to seniors. The FNP will focus on health promotion, disease prevention, and the management of acute and chronic conditions. The ideal candidate will possess a strong foundational knowledge of health across the lifespan, exceptional diagnostic abilities, and a proven talent for building strong, trusting relationships with patients.

Key Responsibilities :

• Conduct comprehensive health assessments, including obtaining detailed medical histories and performing physical examinations for patients of all ages.
• Formulate differential diagnoses for complex health conditions and develop appropriate treatment plans.
• Order, interpret, and evaluate diagnostic tests (e.g., laboratory tests, imaging studies) to inform patient care.
• Prescribe medications and other treatments as appropriate, adhering to state and federal regulations.
• Manage acute illnesses, injuries, and chronic conditions, providing continuity of care.
• Provide health education, counseling, and preventative care services to patients and their families.
• Collaborate effectively and professionally with physicians, medical assistants, front desk staff, and other healthcare team members at various office locations to ensure seamless patient care coordination and efficient clinic operations.
• Maintain accurate and thorough electronic medical records (EMR) in a timely manner.
• Participate in quality improvement initiatives and continuing education to enhance clinical practice.
• Refer patients to specialists as needed and coordinate follow-up care.
• Travel between 3 office locations within Hudson County is required.

Qualifications :

Current, active, and unrestricted Registered Nurse (RN) license in the State of New Jersey.

Minimum of 4 years of nursing experience (RN experience) is required.

Strong knowledge of health principles and disease processes across the entire lifespan.
