• Conduct comprehensive assessments of residents' physical, mental, and emotional health to develop and implement individualized care plans, ensuring optimal patient outcomes.\n
• Provide prescribed medications and treatments, monitor for side effects or reactions, and ensure compliance with treatment plans for chronic conditions, rehabilitation, or post-acute care.\n
• Continuously monitor residents' vital signs, wounds, and overall conditions, providing appropriate interventions and support, and reporting any changes to the healthcare team.\n
• Work with physicians, therapists, and other healthcare professionals to coordinate care and provide education and emotional support to families on patient progress and care requirements.Qualifications : \n
• HI Registered Nurse license\n
• Basic Life Support (BLS) certification\n
