Hospice RN Job Details

Location : Newport News, VA 23601 (Virginia hospice jobs)

Setting : Hospice – Home Health and Facility Visits

Assignment Type : Travel RN

Start Date : December 8, 2025

End Date : March 9, 2026

Duration : 13-week contract

Shift : Day shifts, Monday–Friday, 8 : 00am–4 : 30pm or 9 : 30am–5 : 30pm; every 3rd weekend rotation

Hours per Week : 40

Shift Duration : 8 hours

Caseload : 15–18 hospice patients; 5 points per day

Charting System : Epic EMR

Registered Nurse Qualifications

Active Registered Nurse (RN) license in Virginia or compact state (RN jobs in Virginia)

Minimum 1 year of hospice RN case management experience, including Hope Assessment, pain and symptom management

Current BLS certification (American Heart Association preferred)

Valid driver’s license and reliable transportation

Proficiency with Epic charting system

NIH Stroke Scale and stroke education required

Ability to work independently in home health and facility settings

One supervisory reference required for submission (two for compliance)

Hospice RN Responsibilities

Manage a caseload of 15–18 hospice patients in private homes, long-term care, and assisted living facilities

Conduct Hope Assessments and deliver expert pain and symptom management

Coordinate and provide compassionate end-of-life care, supporting patients and families throughout their hospice journey

Accurately document all care and visits using Epic EMR

Participate in weekly interdisciplinary team (IDT) meetings

Adhere to a point-based visit system (admissions, revisits, evaluations, etc.)

Float to other hospice units as needed based on facility requirements

Maintain compliance with all facility, state, and travel nursing assignment guidelines
