Are you passionate about connecting people with the care they need? We're looking for a dynamic and outgoing Home Health Community Liaison to join our growing team! In this role, you'll be the face of our agency, building strong relationships with referral sources and helping patients gain access to quality home health services.

Key Responsibilities :

• Build and maintain professional relationships with physicians, hospitals, senior living facilities, social workers, and community organizations.
• Promote our home health services through outreach, education, and community engagement.
• Identify and develop new referral opportunities to support agency growth.
• Represent the agency at networking events, health fairs, and other community activities.
• Collaborate with internal teams to ensure a smooth referral and admissions process.
• Track referral sources, activity metrics, and performance trends.

Qualifications :

• Experience in healthcare marketing, community outreach, or a liaison role (home health or hospice preferred).
• Strong communication, presentation, and interpersonal skills.
• Knowledge of Medicare / Medicaid and home health regulations is a plus.
• Highly motivated, organized, and able to work independently.
• Valid driver’s license and reliable transportation required.
• Bachelor’s degree in Marketing, Healthcare Administration, Nursing, or a related field (preferred).
