Admissions Coordinator - Weekends

Overview

Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and customer-focused Admissions Coordinator to join our team in West Palm Beach . This position is onsite in a call center environment.

This full-time position works Friday through Sunday, 9 : 00 a.m. - 9 : 30 p.m. , supporting the admissions process and ensuring a seamless, empathetic experience for patients and families during one of life's most meaningful transitions.

Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24 / 7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

Responsibilities

What You'll Do

• Facilitate the conversion of referrals to admissions through completion of the intake process.
• Provide exceptional customer service to patients, families, and referral sources with empathy and professionalism.
• Communicate effectively with interdisciplinary team members to ensure timely and accurate admissions.
• Maintain accurate records and documentation in accordance with organizational policies.
• Support a compassionate, patient-centered environment that reflects the mission and values of Trustbridge Hospice.
