We are seeking a compassionate, dedicated LPN to join our care team. In this role, you will implement the medical plan of care, oversee Nursing Assistants and High School Student aides, and ensure residents receive high-quality, compliant care. Key Responsibilities
Clinical & Documentation
Utilize Physician’s Computer Company (PCC) EHR / EMR for documentation.Administer medications, treatments, and monitoring (e.g., glucose checks); document and report resident responses.Accurately transcribe physician orders and complete monthly summaries.Update resident care plans based on current condition and collaborate with NAs.Immediately report significant condition changes to the RN Supervisor.Assist physicians during rounds, exams, and treatments.Collect lab specimens and document lab work in PCC.Complete transfer forms, notify family and department heads of transfers, and coordinate communication.Resident Care Oversight
Supervise and direct Nursing Assistants to ensure high-quality resident care.Encourage resident participation in scheduled activities.Monitor NA documentation for accuracy and regulatory compliance.Participate in orientation of new nursing staff and assist in CNA performance evaluations.Monitor and enforce infection control standards.Provide direct resident care when needed.Administrative & Operational
Maintain narcotic counts at shift start / end.Track resident belongings and update inventory.Prepare daily wing reports.Clean and restock treatment trays; maintain work area cleanliness.Submit maintenance requests and report safety hazards.Conduct team conferences and communicate policy / procedure updates.Support quality assurance data collection.Attend care plan meetings, staff meetings, in-services, and required committees.Assist CNAs as needed and perform other duties assigned by leadership.End-of-Life Support
Assist with post-mortem procedures, belongings collection, and departmental notifications.Dress Code
