As a Licensed Practical Nurse (LPN) at Legacy Nursing and Rehabilitation, you will play a crucial role in delivering high-quality, compassionate care that enriches the lives of our residents. Your day kicks off by receiving patient reports and reviewing care plans, setting the stage for your hands-on responsibilities. Throughout your shift, you will engage in a variety of clinical tasks, from monitoring vital signs and administering medications to changing dressings and assisting with personal hygiene. Collaborating closely with Registered Nurses (RNs) and physicians, you'll communicate essential updates on patient conditions to ensure seamless continuity of care. As a vital point of contact for residents and their families, you will provide education and emotional support, fostering a trusting relationship.

Your ability to manage time effectively and demonstrate empathy will be key as you navigate the dynamic, fast-paced environment of our facility, making every moment count for those you serve.

A successful LPN holds a state-approved Practical Nursing program completion and has passed the NCLEX-PN for licensure. Certifications in CPR / BLS are often required, with additional training in geriatric care being a significant asset.

Understanding the specific needs of elderly patients, especially those with dementia or chronic ailments, is crucial. You should be adept in care planning, medication administration, and documentation compliance, ensuring that all practices align with state and federal regulations. Your clinical proficiency must include wound care and infection control protocols.

Strong communication skills will enable you to connect effectively with residents, families, and healthcare teams. Furthermore, your ability to manage time and prioritize tasks in a dynamic environment will ensure that you provide patient-centered, compassionate care to all.

Knowledge and skills required for the position are :

Educational and Licensing Requirements :

  • Completion of a state-approved Practical Nursing program (typically 12-18 months).
  • Passing the NCLEX-PN to obtain licensure.
  • State licensure as a Licensed Practical Nurse (LPN) with renewal as required.
  • CPR / BLS certification often mandatory prior to employment.
  • Additional geriatric care training or certifications may be preferred or required.

Specialized Knowledge for Skilled Nursing Facilities :

  • Geriatric Nursing - Understanding the physical cognitive and emotional needs of elderly patients including those with dementia and chronic illnesses.
  • Long-Term Care Regulations - Familiarity with federal and state regulations (e.g. CMS OBRA) governing SNF operations and patient rights.
  • Care Planning and Coordination - Assisting in the development implementation and monitoring of individualized care plans in collaboration with RNs and interdisciplinary teams.
  • Medication Administration and Monitoring - Administering routine medications observing effects and ensuring proper documentation and compliance.
  • Infection Control Practices - Strict adherence to infection prevention protocols to protect a vulnerable population.
  • Restorative Nursing Techniques - Supporting mobility independence and rehabilitation goals of residents.
  • Key Skills and Competencies :

  • Patient-Centered Care - Delivering compassionate respectful care tailored to each resident's needs.
  • Clinical Proficiency - Skilled in wound care catheter care tube feedings and monitoring for changes in resident conditions.
  • Strong Communication - Effectively communicating with resident's families' physicians and team members.
  • Documentation and Compliance - Accurate and timely charting in accordance with SNF policies and legal standards.
  • Time and Priority Management - Handling multiple residents' needs in a fast-paced routine-driven environment.
