The Opportunity

As we expand and transform the primary care experience, we are looking for a primarily virtual Certified Diabetes Care and Education Specialist to join a proactive, multidisciplinary, technology and data-focused team directed toward innovating, developing, and improving programs. This includes human-centered pathways, protocols, processes, metrics, materials, and policies to support and educate medicaid patients (18 to 64 years of age) living with uncontrolled Type 1 and Type 2 diabetes. This role will support and develop multi-disciplinary teams, as well as provide guidance to complex high-risk members. The Certified Diabetes Care and Education Specialist will have a direct impact on many patients' lives in a meaningful way.

This role requires an energetic, innovative, quick learner who adapts easily to new situations, appreciates challenges, and has a passion to educate, support, and promote self-management of diabetes. You will be a self-motivated and deeply caring individual who enjoys connecting with people, and you identify as a natural teacher, collaborator, innovator, and supporter. You possess a strong ability to connect with people, along with empathetic and altruistic qualities, and are deeply motivated to help develop programs that assist patients in reaching their self-identified health and wellness goals. You demonstrate good judgment, flexibility, creativity, and a problem-solving mindset. In collaboration with the Vayu Health clinical team, you will enhance the level of support available, foster therapeutic relationships, maintain cultural competency, and support a group of patients by building and maintaining trusting relationships.

Education and Licenses Required :

Required :

Bachelor's or Master's degree in Nutrition, Dietetics, or a related field.

Bilingual in English and Spanish preferred.
