Job Description : Home Health Case Manager

Position Summary

The Home Health Case Manager is responsible for coordinating and managing patient care in the home setting. This role ensures patients receive high-quality, individualized plans of care, facilitates communication among interdisciplinary team members, and supports safe, effective transitions throughout the episode of care.

Key Responsibilities

Conduct initial and ongoing patient assessments in the home

Develop, implement, and regularly update individualized plans of care

Coordinate care with physicians, therapists, social workers, aides, and other team members

Monitor patient progress and adjust care plans according to clinical needs

Educate patients and families on disease processes, treatments, and self-management strategies

Ensure timely and accurate documentation in the EMR

Participate in case conferences, quality reviews, and interdisciplinary planning

Ensure compliance with agency policies, state / federal regulations, and Medicare / Medicaid requirements

Communicate changes in patient condition to the care team and physician

Support patient discharge planning and transition of care

May require rotating on-call duties depending on agency needs

Required Qualifications

Active RN license (or specify alternate license if needed : LPN, PT, MSW, etc.)

Minimum 2 years of clinical nursing experience

Strong assessment and critical-thinking skills

Excellent communication, time-management, and organizational abilities

Current driver’s license, auto insurance, and reliable transportation

Ability to work independently in the home setting

Preferred Qualifications

Prior home health case management experience

Familiarity with OASIS documentation (if RN role)

Experience with home health EMRs (HCHB, Kinnser, WellSky, Epic, etc.)

Experience coordinating care for chronic or complex patient populations
