HOME CARE AGENCY RALEIGH, NC

Home Care Agency in Raleigh, NC is looking for a Licensed Practical Nurse (LPNN) to join the agency. As a Licensed Practical Nurse (LPN), you will be responsible for performing initial assessments, ongoing supervisory visits, and supervising in-home aides. We are seeking a leader who can enhance the culture of positivity, collaboration, and accountability.

Responsibilities :

• Perform initial in-home assessments of patients and develop care plan
• Determine ongoing care needs for clients and communicate all findings to Director of Nursing
• Perform in-home supervisory visits and review care plans.
• Assess clients on a regular basis.
• Supervise, train and educate caregivers.
• Assist in conducting caregiver orientation and in-service.
• Review caregivers notes weekly to ensure they are following the plan of care as written.
• Perform caregivers competency skills assessments.
• Investigates and resolves patient / family member concerns regarding patient care
• Maintain compliance with state regulations pertaining to home care agency and assure compliance with company policies and procedures.
