Job Title : Physical Therapist Department : Bloomington Physical Therapy
Reports to : Practice Manager FLSA Status : Exempt (Salary)
Job Summary :
The licensed physical therapist is responsible for evaluation and implementation of
treatment programs. They are responsible for developing and implementing a plan of
treatment for patients with orthopedic / sports or related dysfunction. Physical
therapists are responsible for the education of patients and the public and clinical
education of physical therapy students and physical therapy assistant students. They are
directly responsible for the actions of licensed physical therapy assistants and unlicensed
personnel in the treatment of patients.
Duties / Responsibilities :
- Administers PT procedures in accordance with accepted standards.
- Evaluates, develops, and implements a treatment plan for patients.
- Supervises the treatment of patients by licensed physical therapy assistants, licensed athletic trainers, and on-the-job trained personnel.
- Maintains equipment in a clean and safe condition from treatment to treatment.
- Maintains updated and timely documentation of evaluations, progress notes, and discharge summaries.
- Corresponds regularly with referring physicians.
- Educates patients about their specific problems.
- Educates the public about the role of physical therapy in health care.
- Provides clinical education for physical therapy students and physical therapy assistant students.
- Provides supervision for volunteers considering physical therapy as a profession.
- Maintains all continuing education requirements to maintain licensure in the state of Ohio.

Education : A degree in physical therapy from an accredited university college program.Licensure : Holds an active license in the state of Ohio.Experience : None required beyond entry level educational requirements.Effective Date : 10 / 27 / 2025
