Description

  • Conduct thorough evaluations in inpatient or outpatient environments and develop personalized treatment plans.
  • Implement therapy interventions targeting mobility, strength, balance, and overall function.
  • Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes.
  • Provide education to patients and their families regarding therapeutic objectives, discharge procedures, and at-home exercises.
  • Maintain detailed documentation of evaluations, treatments, and patient progress according to facility protocols and regulations.
  • Engage in interdisciplinary care rounds and conferences to ensure a seamless care continuum.

Education

  • Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution.
  • Certifications

  • Active state licensure as a Physical Therapist .
  • CPR certification preferred.
  • Additional specialized certifications, such as neurology or geriatrics, are advantageous but not obligatory.
  • Skills

  • Prior experience strongly preferred.
  • Capability to treat patients with intricate medical and mobility challenges.
  • Exceptional communication and teamwork abilities.
  • Proficient with digital charting and clinical documentation systems.
