Job descriptionService plan of care is discussed with L & W Legacy Home Care Supervisor on regular basis. Client assignments and reports are received from L & W Legacy Home Care Supervisor. High school graduation or GED required. Certified Nurse Aide certification required as obtained through successful completion of an approved program Criminal history check conducted is required prior to being offered work with this agency CPR / First Aide TB Skin Test within (1) year. Chest x-ray if positive (may require Dr. clearance) One year-full-time experience in personal care or an institutional setting, such as a hospital or nursing home, OR shall have one year full-time experience within the last five years in direct client care in a home care agency setting; OR will consider an applicant with a recent certification as a Nurse’s Aide Exhibits sympathetic attitude towards caring for the elderly Displays maturity and ability to deal effectively with job demands Demonstrates ability to read, write, and carry out directions and has good verbal and written communication skills Evidence of maturity and ability to deal effectively with job demands Attends (8) hours of personal care oriented in services per year Participates in professional meetings when directed Ability to work closely supervised to ensure competence in providing client care Must be able to read & write English Must be physically able to lift, bending and stand for long periods of time Must be able to handle stressful situations due to client’s medical conditions Must adhere to assigned and agreed work schedule Able to drive up to 50 + miles per day Must maintain professional appearance and good personal hygiene

Role and Responsibilities
Ensure quality and safe delivery of home care services.
- Participates in development and implementation of client’s plan of care per home care agency policy and procedure, as appropriate.
- Participates in client case conferences according to L & W Legacy Home Care Agency policy and procedure, if requested.
- Information regarding client’s plan of care is submitted to L & W Legacy Home Care SUPERVISOR in a timely manner.
Implements current Certified Nurse Aide services.
Qualifications and Education Requirements
Physical Demands
Competency Testing :
Certified Nurse Aide is expected to pass competency examination with at least an 80% or better. The content of the competency evaluation of the Agency will include and is not limited to : (if applicable)
