This is a Float position covering territories including Dorchester, Jamaica Plain, Mission Hill, and Roxbury , among others in the region (Paul Revere / Braintree focus). The role requires the ability to interpret care plans, assess patient needs, provide ongoing education, and manage the case from admission through discharge.
