The Lodges operates 12 residential care homes that offer 24 hour assisted living services. Each home accommodates 5 -14 residents comfortably. Requirements
QUALIFICATIONS :
Must be registered and licensed with the State of Minnesota as a RN and must maintain a current and unrestricted licensure.Must live out company's values and maintain values score of 4+.Must be able to lift 20lbs, Stand, Sit, or Walk for long periods.Must have reliable transportation to and from work.Must be able to pass a criminal and / or abuse background check, and drug screeningPrevious experience working with tube feedings, ventilators, tracheostomy preferred.Previous experience working with mental health preferred.Previous experience leading team trainings and training unlicensed staff preferredMust complete Lodges Leadership Academy Courses.5 of 6 sessions are required to graduateMissing session must be completed within 6 monthsMust successfully complete all assigned course pre-work and post-work.Must participate in ongoing quarterly Mentor Functional Workgroups - 3 of 4 annuallyOTHER SPECIALIZED KNOWLEDGE AND ABILITIES :
This position required the incumbent to have :
Possess a sensitivity and aptitude for working with the elderly.Possess skills to communicate effectively with all staff, residents, and family members.Must be able to prioritize and organize work effectively and efficiently.Able to work irregular hours, occasional weekends and evenings.Computer skills in Microsoft Office programs (Word, Excel, etc.), electronic health record systems, and database systems.Ability to maintain a friendly attitude with patients and staff, especially under pressure.Able to manage multiple priorities.ESSENTIAL JOB FUNCTIONS AND TASKS :
Job Function : Work within Assisted Living Licensure including completing essential paperwork and complying with state regulations.
Tasks :
Works to assure facility is in compliance with federal state and local standards and regulations. Is available for license surveys.Works with the Director of Nursing and the Operations Manager to keep forms updated including policies, procedures, Bill of Rights, and related assisted living forms and manualsMeets with Case Workers regularly, as needed.Maintains an up-to-date knowledge of current trends of Assisted Living.Is responsible for investigating and reporting incidents of suspected resident neglect and / or abuse and reporting these findings to the Operations Manager, Director of Nursing, and / or appropriate county officials.Works to maintain privacy and confidentiality of resident, resident's records and resident' living environment.Prepare, record, submit and maintain accurate and timely correspondence and paperwork regarding resident data and billing information as needed.Completes initial assessment of potential residents to assure that their needs will be met, as delegated by the Director of Nursing.Follows the \Assisted Living regulations for providing timely and through nursing assessments for all residents.Assures residents maintain residency requirements, reassess, and recommend alternative placement if necessary.Monitors and maintains resident charts and nursing notes.Communicates any changes in resident's needs or conditions to physicians, resident representative, and county case workers.Monitors and follows thru with ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications and / or medical conditions.Assures all physicians orders are signed and implemented with-in 24 hours.Checks new medications from the pharmacy medication administration by unlicensed staff.Reviews each resident's Medication Administration Record at least monthly and assures accuracy and appropriate signatures.Communicates in staff communication book any information that staff needs to be aware of.Documents disposal of medication per procedure.Assures compliance with regulations and is available for federal and state licensure surveys.Job Function : Provide support and address needs for programs and services.
Tasks :
Maintain resident files and appropriate documents as required.Assess residents on an ongoing basis to assure their needs can be met.Assures residents continue to live safely in their unit, reassess and recommend alternative placement if necessary.Communicates the services to be provided for each resident to staff responsible for delivering the services and assures follow through.Inform personnel of changes in resident's needs and conditions.Maintains communication with residents, their families and staff.Facilitates coordination of resident services with other service providers as needed.Job Function : Participate and function effectively as a team member.
Tasks :
Develop and maintain a positive working relationship with staff, and other Lodges' staff including providing backup to other staff as needed.Articulate information regarding residents to the team.Provide proactive, constructive participation in staff meetings, committees, etc.Assists other departments with special events and family functions.Be compassionate and work with tact and ethical awareness.Be flexible and adaptable to changing situations.Responds appropriately to safety hazards, fire drills and other emergency situations.Job Function : Mentorship
Tasks :
Complete required Leadership Academy training courses and ongoing trainings / meetings including pre and post coursework as assigned.Hosting introductory experience(s) that make mentee(s) feel welcomed and appreciated.Provide guidance, advice, feedback, and support to the assigned mentee(s) on procedures.Provide mentees with all tools to be able to become a better version of themselves.Identify development opportunities for mentee, and regularly provide feedback to Manager.Encourage participation in mentor-mentee bonding initiatives to facilitate mentee's social orientation and engagement.Supporting mentorship during personal and professional challengesLodges Culture champion to mentee(s)Other mentor duties as assigned.Job Function : Lead responsibilities

Responsible for leading and assisting with assisting with special projects outside of the normal scope as assigned by the LALD / Operations Manager and the Director of Nursing.Active participant in interdisciplinary meetings that may be held offsite,Supporting the Director of Nursing with clinical oversightSupporting other locations with clinical oversite and guidanceJob Function : Other duties as assigned.
