Title : RN Clinical Care Coordinator (Full- Time)

Job Summary :

The Field Nurse Case Manager is responsible for planning, coordinating, and delivering home health services. This role emphasizes skilled nursing care, patient and family education, and interdisciplinary coordination. The Case Manager ensures compliance with clinical standards and CMS guidelines while overseeing patient care and supervising field staff within their designated area.

Key Responsibilities :

  • Uphold and model the organization’s Core Values.
  • Conduct comprehensive initial and ongoing assessments of patients and families.
  • Develop and manage individualized Plans of Care; revise as needed.
  • Provide skilled nursing services, including medication administration and specialized procedures.
  • Educate patients and families on health conditions, treatments, and care techniques.
  • Coordinate care frequency and service delivery in line with the Plan of Care.
  • Facilitate timely discharge planning and appropriate referrals for advanced levels of care.
  • Manage assigned caseloads, coordinate weekly case conferences, and participate in on-call rotation.
  • Maintain clear, accurate clinical documentation and communicate updates to physicians regularly.
  • Supervise and evaluate home health aides every two weeks.
  • Ensure necessary medical equipment and supplies are available.
  • Complete supervisory visits for OASIS SOC and recertifications.
  • Collaborate with physicians, interdisciplinary teams, and external health partners.
  • Train and mentor nursing staff as needed.
  • Perform all duties in accordance with the Nursing Practice Act and competency standards.
  • Other responsibilities as assigned by the Clinical Manager or Administrator.

    • Qualifications :

  • Active Registered Nurse (RN) license in the state of practice
  • Current CPR certification
  • Minimum of one year of home health experience
  • Preferred experience : wound care, VAC therapy, ostomy care, IV administration
  • Proficiency with OASIS-E documentation
  • Basic computer skills

    • Skills & Abilities :

  • Strong clinical judgment and initiative
  • Effective problem-solving and decision-making skills
  • Ability to work independently and manage multiple cases
  • Excellent written and verbal communication
  • Compassionate, collaborative, and adaptable
  • Skilled in patient and team communication, even in challenging situations

    • Job Requirements :

  • Valid driver’s license and auto insurance
  • Successful completion of a background check
  • Negative TB test and other required health screenings
  • Ability and willingness to perform field visits
