About The Position

Must-Have :

• Experience with HMOs, IPAs, etc.
• Experience with advance directives, POLST, and protective services processes
• Familiarity with charting systems, specifically Epic

Description :

This position requires a registered nurse specializing in case management.

The candidate must have a minimum of two years of acute care experience.

Working every other weekend is required.

Candidates with prior relevant experience will be accepted.

The nurse will float between units based on departmental needs.

The daily case load may reach up to 25 patients.

The role involves care coordination, discharge planning, and knowledge of CMS regulations.

Nurses must adhere to HIPAA guidelines and manage appeals and denials related to patient care.

Experience in various patient age groups, including adolescent, adult, pediatric, and geriatric care, is essential.

Candidates must be well-versed in admission criteria, benefits eligibility, and needs assessments.

Interdisciplinary support includes collaboration with social services.

Candidates are expected to commit to working three of the five major holiday shifts.

References from supervisory roles covering at least one year of employment in a comparable setting are necessary.

Travelers may be required to float between facilities as needed.

Nurses must demonstrate excellent communication skills and flexibility in their assignments.
