Job Summary and Qualifications

The Registered Nurse (RN) CM is responsible for promoting patient-centered care by coordinating the plan of care forthe patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safeappropriate discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression andtransition of care using established criteria and in conjunction with the multidisciplinary team. The RN CM willcoordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal careand appropriate resource utilization.

St. Davids Medical Center , part of St. Davids HealthCare, is a comprehensive medical facility with three locations : St. Davids Medical Center, Heart Hospital of Austin , and St. Davids Georgetown Hospital .
