Registered Nurse - Home Health

Full-Time Home Health RN - (M-F, 8 : 00 AM-4 : 30 PM)

This position offers regular weekday hours and the opportunity to build lasting relationships with your patients while working collaboratively with a supportive interdisciplinary team.

Schedule : Monday-Friday, 8 : 00 AM-4 : 30 PM

Status : Full-Time

Position Summary :

In this role, you'll oversee a primary caseload of Home Health patients, providing skilled, professional nursing care as prescribed by the physician and in compliance with all regulatory standards. You'll complete comprehensive nursing assessments, develop and implement individualized plans of care, and continually evaluate patient progress to ensure the highest quality outcomes. Collaboration with physicians, team members, patients, and families is key to success in this role.

Primary Responsibilities :

Assessment & Care Planning

• Conduct thorough and compassionate assessments of patients
• Develop individualized care plans in collaboration with physicians
• Identify active and potential needs and adjust plans as appropriate
• Participate in case conferences and interdisciplinary care planning
