Responsibilities

Assessments face-to-face visits for maintaining safety welfare; audit their caseloads

Two different types of case loads (one working with over 60 waiver population; other will be working with the under 60 population)

Facilitate care management and collaborate with appropriate providers or specialists to ensure member has timely access to needed care or services

Provides and / or facilitates education to members and their families / caregivers on disease processes, resolving care gaps, healthcare provider instructions, care options, referrals, and healthcare benefits

Ensure that cost-effective treatment is provided and patient's-rights regulations are followed

Required Qualifications

RN License for OH or Compact RN

2+ years of Home Health exp(No solely hospital background)

MUST be proficient with technology

Waiver knowledge preferred
