You will engage collaboratively with other healthcare professionals and provide exemplary care to our patients. Your responsibilities will include conducting the admission visit at the patient's home, assessing their eligibility for home care services, and developing a tailored plan of care.\n\nQualifications : \n
• Graduation from an accredited nursing program (diploma, associate, or baccalaureate).\n
• Current state license as a Registered Nurse.\n
• Valid state Driver's License.\n
• Minimum of one year experience as a Registered Nurse; two years preferred.\n
• Current CPR certification and Hepatitis consent / declination attestations.\n
