The Role : Licensed Vocational Nurse (LVN)

We are hiring experienced and compassionate LVNs to provide direct one-on-one care to patients in their homes throughout Echo Park City Terrace Maywood Lynwood Vermont Square and Boyle Heights and surrounding areas . Youll work under the supervision of a Registered Nurse (RN) and within a collaborative supportive care team.

Responsibilities Include :

Deliver high-quality hands-on care based on individualized patient care plans

Collaborate closely with RNs and other members of the home health team

Accurately document visits assessments and treatments in a timely manner

Ensure compliance with state regulations and company policies

Support patients in achieving optimal health outcomes through skilled and compassionate care

Qualifications :

Graduate of an accredited nursing program

Current and active California LVN license in good standing

Minimum of 1 year nursing experience (home health experience preferred)

Reliable transportation and a valid California drivers license

Ability to lift bend and move with patients as needed

Strong communication skills and emotional resilience

Proficient with electronic documentation and care coordination
