About the Role
As a valued member of our team, you will engage with patients in their homes or Skilled Nursing Facilities, conducting assessments that can last up to one hour. This role allows you to :
- Perform thorough adult and geriatric assessments, including medical history review, diagnosis and treatment planning, health education, and care coordination.
- Administer point-of-care and lab tests as needed.
- Collaborate with physicians, case managers, social workers, and families to provide comprehensive care.
- Work closely with Primary Care Physicians on patient education and follow-up care.
- Document assessments and care plans according to Matrix guidelines.

Master's Degree or equivalent experience with NP or PA licensure.Current NP or PA licensure in your practice state with prescription authority.Board certification in relevant specialties (Adult, Family, etc.).Current BLS, ACLS, or CPR certification.1 year of experience preferred, but new graduates are encouraged to apply!Valid driver’s license and proof of automobile insurance.Strong computer skills and familiarity with health record software.Excellent verbal and written communication abilities.The Matrix Culture
