Job Title : Clinical Pharmacist – Patient Care
Location : Remote in New York (Tennesse)
Work Hours : 8 : 00 AM – 8 : 00 PM CST, Monday to Friday (rotating shifts; occasional weekends)

Role Overview
The Clinical Engagement Services Center team delivers a variety of clinical services, including :
- Adherence programs
- Diabetes management
- Fraud, Waste & Abuse prevention
- Medication Therapy Management (MTM)
- Retrospective Drug Utilization Review
- STARS adherence programs
The Clinical Pharmacist will primarily conduct telephonic consultations with members to complete comprehensive medication reviews and improve disease state management. Documentation of care and communication with primary care providers is essential. Occasional in-person consultations may be required.
Key Responsibilities
Conduct telephonic consultations and schedule pharmacist appointments for members enrolled in clinical programs.Use prescription claims data to generate clinical recommendations per MTM and Drug Utilization Review protocols.Assess medication regimens for unnecessary or ineffective medications, inappropriate doses, and adverse drug reactions.Provide patient education on medication regimens and disease states.Document patient care notes and recommendations clearly and professionally.Stay updated on emerging clinical trends.Participate in departmental meetings to improve programs and processes.Meet productivity and performance expectations.Support special projects as needed.Perform other duties as assigned by Operations Manager.Required Qualifications
Bachelor's Degree in Pharmacy or PharmD.Current and unrestricted Pharmacist license in state of residence.Strong knowledge of disease state management guidelines and MTM Core Elements :Medication Therapy Review (Comprehensive & Targeted)Personal Medication List (PML)Medication-Related Action Plan (MAP)Interventions and ReferralsDocumentation and Follow-upAbility to work independently in a remote, cross-functional team environment.Proficiency in MTM platforms, databases, and technology.Flexible to work rotating shifts between 8 : 00 AM – 8 : 00 PM CST, Monday–Friday; occasional weekends.Preferred Qualifications
PharmD degree.Bilingual in Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese.2+ years of pharmacy experience in retail / community or PBM (new graduates considered).Additional pharmacist licenses in AL, ID, KY, LA, MD, MN, OR, TN, or VT.Prior call center experience.ACPE MTM Specialist certification.Certified Geriatric Specialist.Board Certified Pharmacist.
