Schedule : Mon–Fri, 8 : 00 AM–5 : 00 PM (40 hrs / week, on-call if needed)

Description :

Provide in-home physical therapy care using HCHB / OASIS. Manage patient visits, treatment plans, documentation, and coordination with care teams.
