Reports to the Nurse Manager / Director and is responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting. Participates in identifying and meeting learning needs for self. Attends education programs based on identified learning needs. Assesses readiness to learn of the patient, family, and caregivers. Evaluates expected patient outcomes and seeks guidance as needed in revision of the plan of care. Communicates relevant information to promote continuity of care. Demonstrates competence in the care of patients with complex problems, considering physical, psychosocial, educational, and safety aspects of care. Performs bedside point of care testing as required for patient care. Utilizes nursing standards, policies, and procedures in the delivery of care. Organizes and prioritizes care according to patient and family needs. Consults with appropriate resources regarding patients with complex care issues. Develops goals to promote professional growth or minimize limitations. Participates in the achievement of unit goals and performance improvement activities. Uses communication systems effectively. Promotes cost-effective care and gives feedback to co-workers. Assesses and synthesizes data from complex situations to determine priorities for care. Develops relationships with families to promote their ability to advocate for the patient and their own needs. Collaborates with patients and families to prepare or update the plan of care. Identifies and addresses cultural and ethnic issues in planning patient care. Demonstrates ability to prioritize tasks for patients with complex problems. Reads journals to stay informed about relevant studies or articles and shares ideas with staff.
