Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel.

Position Details :

Mercy Clinic 65 Prime Plus - North County 637 Dunn

Schedule : Monday - Friday, 0800-1630

No weekends or holidays!

Mercy 65 Prime+ Clinics are a new primary care service designed specifically for Medicare beneficiaries. Patients receive extended office visit times and closer monitoring of chronic conditions like high blood pressure, diabetes, and heart disease. As a Primary Care Nurse (PCN) in this clinic, you will work closely with the providers to triage incoming calls / messages, educate patients, follow-up with patients, and assist with various clinic tasks as needed.

LPN Qualifications :

• Education :   Diploma in Practical Nursing
• Licensure : Active LPN license
