Job Summary :

• The Midlevel Provider will deliver high-quality, patient-centered medical care to residents in both long-term care and short-term rehabilitation settings. This role focuses on promoting optimal health outcomes, managing chronic conditions, and supporting recovery for post-acute patients. The provider will collaborate closely with attending physicians, nursing staff, therapy teams, and families to ensure comprehensive care.

Salary Range :   $120,942.45 - $156,476.75
