Overview : The Afterhours Care RN Manager plays a crucial role in addressing and coordinating patient and family needs during after-hours or alternative scheduling. This position ensures primary control and professional management of patients assigned during working hours.
- Manage patient needs by conducting routine visits, admissions, death visits, on-call or after-hours visits, and other duties as assigned per the patient’s care plan.
- Provide after-hours coverage for designated teams as part of the interdisciplinary team, facilitating communication between physicians, patients, families, and the hospice team.
- Execute clinical administrative tasks including workflow completion, auditing, survey readiness activities, and other responsibilities as assigned.
About You : To thrive in this role, you should meet the following qualifications :
Current RN licensure in the state of employment.Graduate of an accredited nursing program.Minimum of one (1) year of nursing experience in acute care hospitals, oncology, long-term care, hospice, or home health.For state-specific requirements, state rules will apply and supersede the above.Licenses, Certifications, and Registration :
Current nursing license in the respective state.CPR Certification (in-person).Must have current automobile insurance, a valid driver’s license, and access to a reliable, insured vehicle for work-related duties.Specialized Knowledge and Skills :
Understanding and compliance with hospice principles and best practices per industry standards and regulations.Expertise in nursing practices, with the ability to perform thorough and accurate patient assessments.Ability to address the unique needs of hospice patients and families from diverse socioeconomic backgrounds.Knowledge of the roles of various disciplines providing hospice services and familiarity with community resources.Strong time management skills and the ability to adapt priorities based on patient and business needs.Collaborative skills to work closely with operations, clinical teams, and other departments to enhance processes and achieve high-quality outcomes.Exceptional oral and written communication skills for effective interaction with individuals of varied backgrounds.Maintain confidentiality regarding all patient and associate information.Proficiency in technology, including mobile devices and software like Microsoft Office suite and Electronic Medical Records systems (HomeCare HomeBase).Demonstrated leadership and accountability in supervising, teaching, evaluating, and empowering team members for optimal patient outcomes.Commitment to maintaining high ethical standards and business integrity.This position is classified as safety-sensitive.Personal Traits and Qualities :
