Unlock your potential at Mission Hospital! Job Summary and Qualifications Schedule : Monday-Friday 8am-430pm with a rotating weekend every 4 weeks. This position is onsite at Mission Hospital. The Registered Nurse (RN) CM is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The RN CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. What qualifications you will need : Required Experience : 2 years in case management OR 3 years of experience in a clinical setting. Required Education : Associates Degree in Nursing Preferred Education : Bachelor of Science in Nursing or bachelor’s degree in a related field. If RN, effective 3 / 1 / 18 Bachelor of Science in Nursing OR enroll in a BSN or advanced degree program within 6 months AND obtain a BSN within 4 years of hire date into RN position. Previous signed BSN commitment letter OR grandfathering will be accepted between departments and Mission Health entities. Exemptions may be approved with CNO System Council approval. Preferred Education : Masters Degree in Nursing Required License : If RN, must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse License Compact) license may apply, licensee should confirm with NCBON.
