Job Summary :

Provides direct palliative care to patients and their families while functioning within a medically approved plan of care. The Nurse Practitioner will deliver services under the direction of the interdisciplinary team and Chief Medical Officer or Hospice Physician.

Job Duties and Responsibilities :

Essential Job Functions :

  • Fosters and maintains an effective working relationship between co-workers, patients, and other professional members of the community
  • Follows an individualized Plan of Care established by the Interdisciplinary Team for each patient
  • Assists the Chief Medical Officer and / or Hospice Physician in the oversight of patient care by providing direction for orders and interventions to optimize patient outcomes
  • Assists the RN Case Managers with identifying treatment goals and interventions for patients
  • Gathers, reports, and analyzes patient care data and implements care accordingly
  • Performs face-to-face visits for patients within a specified time frame
  • Assists clinical team with family coping by providing advanced medical insight to patient conditions and treatment options
  • Capable of giving adequate reports on patient's condition to the Chief Medical Officer, Hospice Medical Director, RN Case Managers, and other members of the interdisciplinary team, as needed
  • Reports unusual conditions, observations, and pertinent information to the correct interdisciplinary team members
  • Understands / provides basic principles of palliative care / pain management within the Scope of Practice
  • Complies with all documentation policies and procedures
  • Completes appropriate documentation in a legible and timely manner
  • Completes physician orders and medication orders, within scope of practice, in a timely manner
  • Utilizes appropriate chain of command
  • Supervises and delegates duties to all members of the nursing team as needed
  • Participates in Interdisciplinary Team Meetings and other Performance Improvement Activities, as needed
  • Signs death certificates under NRS guidelines
  • Provides intervention and support as needed related to terminal illness and loss
  • Provides emotional support and makes appropriate referrals to other team members
  • Assesses and addresses depression and fear associated with terminal illness for both the patient and any family members or loved ones
  • Facilitates communication among patient, family, and hospice team
  • Identifies caregiver strain which may necessitate respite care relief and / or support
  • Functions as a liaison between RN Case Manager, Admissions RN and Chief Medical Officer or Hospice Physician
  • May participate in the competence evaluations of nursing staff and other nurse practitioners
  • Participates in community education, as requested, by the Administrator, Director of Nursing or Chief Medical Officer
  • Maintains working knowledge of pharmaceuticals for management of patient symptoms including, but not limited to, pain, shortness of breath, nausea, constipation, and agitation
  • Follows Nevada Rule AB474
  • Assures patients continue to meet hospice criteria to remain on service and suggest re-certification or discontinuation of service to the Chief Medical Officer or Hospice Physician, as needed
  • May be required to work some nights and weekend on-call
  • Consults appropriately with Chief Medical Officer or Hospice Physician if uncertain about management of a disease, treatment options, or if required interventions beyond the knowledge and expertise of the nurse practitioner
  • Other duties as assigned

Specific to Job Position :

  • Adheres to patient rights, abuse reporting and confidentiality policies
  • Attends scheduled staff meetings and / or in-service trainings
  • Adheres to departmental dress code requirements related to uniform, personal grooming, and other attire
  • Demonstrates reliability by making every attempt to report to work on time, providing proper notification for absence or tardiness and participating in required meetings
  • Demonstrates ability to problem-solve and make decisions by assessing situations and considering alternatives, seeking guidance and direction as necessary, utilizing proper chain of command
  • Demonstrates effective time management skills by coordinating activities to achieve maximum productivity and efficiency, willingness to adjust or revise work schedules as requested
  • Assumes responsibility for on-going personal development and continuing education
  • Stays current with all mandatory agency and regulatory requirements
  • Completes assigned projects within specified time frames
  • Physical / Safety Requirements :

  • Must have the strength and endurance required for standing, walking, sitting for extended periods of time
  • Must be able to lift up to 25 lbs. occasionally
  • Must utilize appropriate durable medical equipment as needed in a safe manner
  • Must use appropriate body mechanics as needed for personal use and safe patient care
  • Category 1 for potential exposure to blood / body fluids. (Requires the performance of procedures or other tasks in the work routine that involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues, or a potential for spills or splashes of them. Protective measures should be used while engaged in Category 1 tasks with exposure potential.)
  • Utilizes appropriate PPE as needed
  • Follows the designated plan of action in the event of a fire or other emergency / disaster
  • While performing the duties of this job, the employee is regularly exposed to fumes or airborne particles, work-related stresses, and driving hazards
  • This individual in this position may required to drive to meetings with clients, health care professionals, or patient's homes
  • The noise level in the work environment is usually minimal
  • The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.

  • Qualifications and Experience :

  • Must be currently licensed as a Family Nurse Practitioner or Pediatric Nurse Practitioner by the State Board of Nursing in the State of Nevada
  • Prefer 2 years experience as a Nurse Practitioner
  • Prefer prior experience in hospice or palliative care
  • Must successfully complete a background check pursuant to NRS 449.188 and NRS 179A.100, a pre-employment physical and drug screen, verification of references and previous employment (may include verification of education) and driving history (MVR)
  • Must read, speak, write, and understand the English language
  • Maintain BLS Certification
  • Maintain active APRN licensure with the Nevada State Board of Nursing
  • Maintain active NPI and DEA registration and provides such information to the company
  • Maintain proper licensure and active status to prescribe non-controlled substances and controlled substances through the Nevada State Board of Pharmacy
  • Maintain active enrollment in the Nevada State Prescription Monitoring Program
  • Must not be exempted from working with Medicare patients
  • Must be able to handle frequent interruptions
  • Must be able to cope with emotionally charged situations encompassing, patients, families, and other caregivers
  • Must be available for prescheduled on-call hours, weekends, and Holiday
  • Must possess reliable transportation, maintain current auto insurance and a valid driver's license
