We are excited to invite a PRN Occupational Therapist (OT) to join our devoted rehabilitation team. This role is ideal for a flexible and skilled therapist eager to make a meaningful impact while enjoying the freedom of PRN scheduling, with the expectation to work on-site.

Position Summary :

As a PRN Occupational Therapist, your primary responsibility will be to evaluate and treat residents, enhancing their daily living capabilities, promoting independence, and ensuring the achievement of rehabilitation goals. You will work in close collaboration with a multidisciplinary team to deliver high-quality, person-centered care.

Key Responsibilities :

• Conduct comprehensive evaluations of residents’ physical, cognitive, and psychosocial needs.
• Develop and implement individualized treatment plans aimed at improving residents’ functional abilities.
• Provide therapy interventions, including training in adaptive techniques and the application of assistive devices.
• Monitor residents’ progress and modify therapy plans as necessary to achieve desired outcomes.
• Collaborate with the care team, residents, and families to define and support therapy goals.
• Educate residents and their caregivers on therapeutic strategies and techniques to foster independence.
• Maintain thorough and accurate documentation of therapy services, evaluations, and progress notes.
• Adhere to all state and federal regulations alongside facility policies and procedures.
