Job Description :
- Treatment Plan Development : Develop effective treatment plans and obtain necessary approvals from referring physicians.
- Patient Care : Treat patients according to physician treatment plans.
- Supervision : Supervise Physical Therapy Assistants in direct patient care and related activities, following state practice acts.
- Collaboration : Communicate with supervisors and other health team members regarding patient progress, problems, and care plans.
- Meetings Participation : Attend Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
- Documentation : Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation in accordance with PT Board requirements and policies.
- Family Instruction : Instruct patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge.
- Discharge Planning : Participate actively in discharge planning processes.
- Recommendation of Equipment : Recommend and facilitate the ordering of necessary durable medical equipment to enhance patient mobility and independence.
- Equipment Maintenance : Report issues with department equipment to ensure it is maintained in good working order.
Required License or Certification :
- Valid Physical Therapist License
