You will be part of an interdisciplinary care team of social workers, chaplains, nurses, physicians, and certified nursing assistants all working together to improve people's everyday lives when they need help the most. You will be an advocate by providing education and resources to patients and their families, being out on the road and seeing our patients / families in their personal living environments in this beautiful rural community.

Who Should Apply?

• Social Workers with a minimum of one year experience in healthcare.
• Experience working (paid or volunteer) with the elderly preferred.
• Excellent interpersonal and communication skills.
