Compensation : $95,000 - $130,000 per year + Incentive Visits

Position Summary

In this full-time RN Case Manager role, youll assess clients health needs, provide skilled nursing care, and guide caregiver training across the Twin Cities Metro Area. Youll move around to help with caseload fluctuations across designated territories to ensure a timely initiation of care and progress clients through their care plan.

Key Responsibilities

• Deliver nursing services, client assessments, and ongoing care
• Train and mentor caregivers to uphold high standards of service
• Engage in process improvements for continuous quality enhancement
• Rotate in the on-call schedule
