Job Details

What You Will Be Doing

Provide direct nursing care and administer treatments / medications as prescribed

Visit residential homes to assess health needs and ensure quality care

Develop personalized care plans and advocate for residents’ health services

Coordinate with families, healthcare providers, and agencies for seamless care

Train and certify staff in AMAP, G-Tube, and Diabetic management

Maintain accurate documentation in electronic medical records (Precision Care)

Participate in interdisciplinary team meetings and regulatory training sessions

On-call nursing coverage as needed

Skills You Need

NYS Registered Nurse certification

1+ years working with individuals with intellectual / developmental disabilities preferred

Basic computer proficiency

Strong communication and teamwork skills

Ability to document and report clinical activities accurately
