About the Role

Join Ryse Health's high-performing clinical team to improve the lives of patients with diabetes and related endocrine conditions. This hybrid role offers 2 days per week remote work, meaningful patient impact, and collaboration with a team that values innovation and autonomy.

What You'll Do

• Deliver exceptional patient care in-person and virtually, focusing on diabetes and endocrine issues.
• Conduct patient phone calls, messages, and remote data analysis to drive care decisions.
• Manage panel-level care plans for high-priority patients.
• Collaborate with physicians and the clinical team to optimize patient outcomes.
• Participate in shared call responsibilities.
• Build trust-based relationships with local primary care providers and team members.
