Registered Nurse Transitional Liaison

Full Time | Monday–Friday | 8 a.m.–5 p.m.

What You’ll Need

Graduate of an accredited RN program

3+ years of clinical nursing experience

Current RN license in the state of practice

Valid driver’s license, reliable transportation, and current auto insurance

Hospice experience is required

How You’ll Make an Impact

As a Transitional Liaison RN, you will :

Identify patients who may benefit from hospice services using established clinical guidelines

Support and guide patients, caregivers, and families in understanding eligibility, services, and care options

Conduct initial physical, psychological, social, and spiritual assessments

Collaborate with internal teams to ensure proper documentation, smooth transitions, and continuity of care

Serve as a resource and educator for home health teams and referral partners

Help families understand hospice philosophy, services, and what to expect during a transition

Advocate for patients' goals, preferences, and dignity at every step
