As a Home Care Registered Nurse (RN) at Trinity Health At Home, you'll deliver one-on-one, high-quality care to patients in the comfort of their homes. Using advanced technology and your clinical expertise, you'll assess, plan, and manage individualized care that promotes healing and independence.?

What You'll Do :

Provide skilled nursing care, assessments, and interventions in the home

Serve as case manager and patient advocate

Collaborate with interdisciplinary care teams

Educate patients and families on care plans and self-management

Position Highlights :

Schedule : Weekend / Baylor (work 36 hours, get paid for 40 hours!)

Location : Grand Rapids and surrounding areas

Status : Full-time with benefits

Setting : Home health - travel required

Minimum Qualifications :

Graduate of an approved Nursing education program, BSN preferred.

Current Registered Nurse licensure in the state of practice.

Must have a minimum of one (1) year professional nursing experience.

Home Health, Med-Surg, ER, Urgent Care, ICU, Critical Care, Progressive Care, or Step-Down Unit experience

Strong computer and technology skills required.

Must have current Driver's license and reliable transportation.
