Job Title : Physical Therapist

• Ability to work collaboratively with other healthcare providers

Description :

Seeking a full-time physical therapist in a hospital setting with a focus on outpatient rehabilitation and inpatient coverage.

Responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.

Gains confidence and cooperation from patients and their families through competent patient assessment, attentive monitoring, and care.

Effectively communicates with other healthcare providers to ensure optimal patient care.

Adheres to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering patient care.

Provides clinical supervision of assistants and aides and serves as a clinical instructor for assigned students.

Plans, prepares, and implements individually designed physical treatment programs to maintain, improve, or restore physical functioning.

Completes and documents patient evaluations and tests per established standards and regulatory requirements.

Develops and updates the patient's Plan of Care based on patient assessments and identified problems.

Assesses and provides treatment for patients per their Plan of Care.

Completes all documentation related to charge capture per established standards.

Coordinates patient care effectively with other care providers and support team members.

Maintains compliance with departmental and organizational requirements and responsibilities.

Works flexible shift assignments as required.

Participates in quality improvement processes and interdisciplinary projects.

Maintains a clean, neat, and safe working environment.

Inspects and uses equipment according to manufacturers' guidelines.

Maintains strictest confidence of all patient Protected Health Information (PHI).

Documents services in accordance with state and federal regulations.

Retains certification and completes mandated continuing education requirements to meet accreditation and licensure requirements.

Graduate of an accredited physical therapy graduate program is required.
