Responsibilities
- PATIENT ASSESSMENT AND PLANNING :
Assess patient eligibility for specific service needs, developing individualized integrated action plans.
Refer and connect patients to resources, monitoring adherence and providing necessary follow-ups.
Complete detailed reports on needs assessment and case management activities.
Provide crisis and de-escalation techniques.
Attend required meetings, including supervision, treatment team meetings, and quarterly in-service.
KNOWLEDGE SKILLS, AND ABILITIES
Qualifications
EXPERIENCE
Required : 1 year of experience working with adults in a human service setting.
Preferred : Prior experience with harm reduction.
EDUCATION :
Required : Bachelors Degree in a related Behavioral Science field.
