What you'll do

• Provide full-spectrum outpatient primary care for adults and adolescents : wellness, chronic disease management (HTN, DM, HLD, COPD / asthma), women's health basics, behavioral health integration, and urgent-care-level complaints.
• Close care gaps and optimize quality measures (preventive screenings, immunizations, chronic care targets) with strong MA / NP and central care-coordination support.
• Perform in-scope procedures as trained (e.g., EKG interpretation, simple skin procedures / cryotherapy, joint injections).
• Practice in eClinicalWorks (smart sets, templates, and macros provided) with
• Collaborate cross-site (Fresh Meadows, Great Neck, Williston Park, Astoria, and Brooklyn) and escalate specialty needs through our referral network.
• Contribute to practice growth through exceptional patient experience, community reputation, and efficient access (same- / next-day slots).
