GENERAL STATEMENT OF DUTIES

Elbert County Public Health Department is seeking a compassionate, dedicated, and highly skilled Nurse Practitioner (NP) to provide primary and preventive health care services to individuals and families across their lifespan. The NP will function as an integral part of a multidisciplinary team to deliver high-quality care in a public health setting, with a strong focus on underserved and vulnerable populations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Conduct comprehensive health assessments including physical exams, medical histories, and screenings
• Diagnose, treat, and manage acute and chronic health conditions
• Order, perform, and interpret diagnostic tests and lab results
• Prescribe medications and non-pharmacologic treatments in accordance with state laws and collaborative agreements
• Provide patient education on disease prevention, healthy lifestyle choices, and chronic disease management
• Collaborate with physicians, nurses, case managers, social workers, and public health staff
• Participate in health promotion and disease prevention initiatives
• Maintain accurate and up-to-date electronic health records (EHR)
• Provide oversight and assist in developing clinic and public health policies and procedures
• Adhere to clinical protocols, policies, and standards of care
• Participate in quality improvement, data reporting, and public health research initiatives as needed
• Support emergency response efforts and community outreach activities
• Maintain compliance with all HIPAA and confidentiality requirements
• Completes job specific training as required
• Various additional duties as assigned
