Duties
The High-Fidelity Children's Wraparound Initiative is to serve the population of most vulnerable children and youth who meet the following conditions to be eligible for this service :
- 3-21 years of age; and
- Diagnosis of Serious Emotional Disturbance (SED), and or an SMI : and
- Multi-system involvement, i.e. involvement with two or more systems including Juvenile Justice, Protective Services, Special Education or Behavioral Health; and
- At-risk of or in an out of- home placement OR previous out-of-home placement, incarceration, or acute hospitalization within a two-year period prior to evaluation; and
- Functional impairment in home, school or community, as measured by the Children and Adolescents Needs and Strengths (CANS).
Using Wraparound values, principles and practices, the Wraparound Facilitator provides intensive, supportive interventions for identified families. Such interventions include running team meetings, coordinating care with other agencies, developing and utilizing informal / formal supports, identifying and making use of family strengths, etc. The goal of Wraparound services is to help the child / youth and family realize their vision and increase their autonomy from formal systems.
Essential Skills
The Wraparound Facilitator Must have the following skills :
Engagement Skills-Work closely with the family and other team members to help them understand the purpose and value system of the Wraparound approach and to encourage them to effectively participate in the process.Planning Skills- Organize team members for planning team and engage team members in developing an overall plan of care and guide the team members towards open, strengths-based, family friendly problem solving.Implementation Skills-Facilitate communication among all team members in planning meetings regarding the follow through, timeliness and outcomes of planned interventions.Transition Skills-Analyze progress toward the meeting of service goalsOther Essential Skills-exercise tact and sensitivity in performance of job dutiesEducation & Experience
Master’s degree in social services and one year of experience with the target population, orBachelor's degree in social services and two years of experience with the target population, orAssociate’s degree in social services and four years of experience with the target population, orHigh School diploma or high school equivalent with six years of experience with the target population.Experience, lived or paid, with the target population includes working in individual, family and children’s behavioral health services and working with abused, neglected and traumatized children, adults and / or families.Position Requirements
Wraparound facilitators are expected to complete the Wraparound certification path credentialed by the New Mexico Credentialing Board for Behavioral Health Professionals. Re-certification every two years as a Wraparound facilitator. The Wraparound Facilitator is expected to meet the following :
Manage a case load of a maximum of 10 familiesDedicated time to Wraparound engagement and activitiesAttend all required trainingsConditions of Work
Hiring and promotion will be accomplished in compliance with the GCLC's Personnel Policies and Procedures. This position is exempt under the Fair Labor Standards Act. This position requires a minimum of 40 hours per week generally Monday through Friday. Hours or days may vary according to need. Travel is required of employees for delivery of services, training, or conferences. Personal transportation is mandatory.
