Job Title – Registered Nurse (RN) Quality Coordinator | Case Manager

Location : Seattle, WA 98112

Contract : 3 + Months of Contract (Possibility of Extension)

Shift : 08 : 00 AM – 05 : 00 PM

Pay Rate :

Local Pay Rate : $60 / hr on W2
Travel Pay Rate : $,2550.30 / Weekly

Job Summary :

The Quality Coordinator supports clinical quality initiatives within primary care and cardiology settings. This role focuses on improving hypertension management, care coordination, patient outreach, and telehealth-based quality interventions. The coordinator collaborates with providers, care teams, and leadership to drive performance improvement and ensure regulatory compliance.

Key Responsibilities :

Support clinical quality programs within primary care and cardiology departments.

Conduct hypertension-focused outreach, monitoring, and quality assessments.

Participate in care coordination and case management activities.

Assist with medication management workflows and follow-ups.

Utilize telehealth tools to support patient interactions and quality initiatives.

Monitor, track, and report quality metrics and clinical outcomes.

Collaborate with clinical staff to ensure best practices and adherence to guidelines.

Support audits, documentation, and data reporting as required.
