Patient Flow Facilitator-LVN. 1000-2030 shift.

Job Summary : The Patient Flow Facilitator-Licensed Vocational Nurse (PFF-LVN) is IV Therapy and Blood Withdrawal Certified in California and carries out patient care responsibilities under the supervision of a Registered Nurse (RN) and within the LVN Scope of Practice and in accordance with Loma Linda University Medical Center (LLUMC) and department established policies, procedures, protocols, techniques and guidelines. The PFF-LVN does not have a physical home unit, but instead, functions out of a home department and readily floats to all units of the hospital for full or partial shifts, or the hospitality / discharge lounge, the primary focus is patient flow. Assists with patient admissions, transfers and discharges by transporting patients to assigned units or transporting discharge patient to designated area, assists the RN with gathering data for the development of the care plan on admissions. Provides discharge teaching and reviews the after-visit summary with patient, family or caregiver as requested. Seeks information and clarification of duties and responsibilities when necessary. Exhibits interpersonal skills that portray professionalism and maximize excellence in customer service, safe patient care and professionalism. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed.

Education and Experience : Graduate of a school of Vocational Nursing. Minimum of one year experience as an LVN required. Experience caring for adult medical and surgical patients preferred. Successful completion of a Basic Arrhythmia Recognition and Treatment (BART) course required within six months of hire.

Knowledge and Skills : Knowledge of the California LVN scope of practice required. Demonstrates competencies in clinical nursing activities as outlined by the LVN board. Ability to apply theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in area of specialty. Basic knowledge of healthcare regulations and accreditation required. Exhibit professional behavior at all times. Able to : read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer and software programs necessary to the position; troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and / or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place / patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch.

Licensures and Certifications : Active California Licensed Vocational Nurse (LVN) license required. Basic life Support (BLS) certification issued by the American Heart Association required. California IV Therapy and Blood Withdrawal certification required.
