Position Summary
The Home Health Physical Therapist Assistant (PTA) provides physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist (PT). The PTA assists in implementing care plans to help patients regain or improve mobility, strength, balance, and overall physical function, promoting independence and quality of life in the home setting.
Key Responsibilities
- Provide physical therapy treatments as outlined in the plan of care established by the supervising Physical Therapist.
- Observe, record, and report the patient's progress, responses, and any changes in condition to the supervising PT and / or clinical team.
- Instruct patients and caregivers in exercises, safe mobility techniques, and proper use of assistive devices.
- Communicate effectively with the care team, including nurses, therapists, and physicians, to ensure coordinated care.
- Document all visits, interventions, and patient progress in accordance with agency policy and regulatory requirements.
- Maintain compliance with all federal, state, and agency standards, policies, and procedures.
- Participate in patient care conferences, in-services, and performance improvement initiatives as needed.
- Ensure a safe home environment for patient care and report any safety concerns.
Qualifications
Graduate of an accredited Physical Therapist Assistant program.Current, valid state license to practice as a PTA.Current CPR certification.Minimum of 1 year of clinical experience (home health experience preferred).Valid driver's license, reliable transportation, and proof of auto insurance.Strong communication, documentation, and time management skills.Ability to work independently and manage a flexible schedule.Physical Requirements
Ability to lift, move, and assist patients as needed.Frequent standing, walking, bending, and reaching.Must be able to travel between patient homes.Preferred Skills
Experience with EMR systems (e.g., Homecare Homebase or similar).Knowledge of Medicare and home health regulations.Compassionate and patient-centered care approach.
