Description of Work Assignment : General Neurology outpatient clinic, inpatient and ED consults are needed during office hours.

Location of Work Assignment : NY- Plattsburg (CVPH)

Breakdown of Clinic Days / Sessions and / or OR Days : Monday Friday (2 sessions per day)

Schedule of Days / Times Needed To Work / Shift Hours : Monday to Friday 8 5 PM

Procedures Provider Will Be REQUIRED To Perform : Botox, EMG

Procedures Preferred : BOTOX and EMG
