Field Nurse Practitioner

Locations : Boise, ID; Palm Springs, Bakersfield, Fresno, CA; Middletown, New Haven, Litchfield, Willimantic, Rockville, New London, CT

Molina Healthcare is seeking experienced Nurse Practitioners to provide in-home and community-based care, focusing on preventive services and health screenings. Work environments include patient homes, nursing facilities, and pop-up clinics.

Key Responsibilities :

Conduct comprehensive medical assessments

Provide care coordination and treatment planning

Perform face-to-face visits (home, SNFs, community sites)

Use Epic EMR in the field

Travel within a 60-mile radius; up to 120 miles occasionally

Attend mandatory 5-week training

Requirements :

Must be comfortable working autonomously in non-traditional settings

Able to manage diverse patient populations

Strong documentation and diagnostic skills

Schedule : 45 minutes per visit

6 9 visits / day depending on shift

No follow-ups or ongoing care
