We are seeking a qualified PRN Registered Nurse for our Hurricane office and will be required to provide services to the counties of Putnam, Wayne, Cabell and Kanawha.
We provide all of our field clinicians with the necessary PPE for their safety during COVID-19.
Personal nurses to provide care for adult and pediatric patient population in their homes.

ESSENTIAL JOB FUNCTIONS / RESPONSIBILITIES
Evaluates patients referred for home care to determine appropriateness for admission. Accepts or rejects patients based on Agency’s admission criteria.
Establishes a home care plan including assessment of the appropriateness of the requested services.
Assures that Agency is ready to meet the patient’s needs by collaborating with Agency personnel on a regular basis.
Explains Agency’s policies and services to the patient and responsible family members during initial home care assessment.
Completes documentation accurately, and submits to agency office in a timely manner including OASIS documentation.
QUALIFICATIONS
Graduate of an approved school of nursing.
Currently licensed, in good standing, to practice in West Virginia as a Registered Nurse.
One year community health nursing experience or equivalent professional experience.
Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS.
Complies with accepted professional standards and practice.
Skilled in interpersonal relationships.
Demonstrates strong verbal and written communication skills as well as critical thinking skills
Demonstrates professional behavior, attitude and flexibility.
Able to function independently, with minimum supervision
